Simple care often comes before a paid procedure

Gentle, steady movement helps many sore joints. It keeps nearby muscles working and may ease stiffness after rest. You don't need a hard workout to begin.

Choose movement that feels manageable during and after you do it. A brief walk or easy home exercise may be enough. If the ache stays worse, your doctor can change the exercise or shorten it. That keeps home care within a fair limit.

Heat, cold and support can make movement easier

Heat before activity may loosen a stiff joint. Cold afterward may settle soreness from extra walking or yard work. A brace or cane can take pressure off the sore side. You'll learn which choice helps by checking how you feel later.

Medicine may help, but your other health needs matter. Some pills can affect the stomach, kidneys or heart. Your doctor can name a safe type and amount. Don't borrow a pill that was meant for someone else.

Clinic care may include therapy, a shot or surgery

Physical therapy can build strength and help you move more safely. A doctor may discuss a brace, medicine or a steroid shot into the joint. Surgery can still be worth discussing when daily life remains hard. You'll know exactly which kind of care is under discussion.

The 2019 ACR/Arthritis Foundation guideline makes STRONG recommendations for exercise, weight loss in people with overweight or obesity, self-efficacy and self-management programs, tai chi, cane use, tibiofemoral bracing, topical NSAIDs, oral NSAIDs and intra-articular glucocorticoid injection in knee OA. Notably it does NOT strongly recommend any biologic injectable.

OARSI 2019 designates arthritis education plus structured land-based exercise (with or without dietary weight management) as CORE treatments for knee OA and strongly recommends topical NSAIDs (Level 1A), while strongly recommending AGAINST oral and transdermal opioids (Level 5). The treatments with the strongest evidence in this condition remain the least dramatic ones.

The Cochrane review of exercise for knee osteoarthritis found high-quality evidence that land-based therapeutic exercise provides short-term benefit in pain and physical function, sustained for at least 2-6 months after the programme ends, with mild transient soreness the only reported adverse effect across 45 trials. It is the best-evidenced treatment for this condition and it costs nothing per injection.

A randomized trial in the New England Journal of Medicine compared physical therapy against intra-articular glucocorticoid injection for knee osteoarthritis and found physical therapy produced better WOMAC outcomes at one year. When a clinic offers an injection, the comparator that matters is not 'nothing' - it is a course of supervised exercise.

  6. In a 2-year RCT, intra-articular triamcinolone given every 12 weeks for knee OA produced significantly GREATER cartilage volume loss than saline, with no significant pain benefit. The most widely used joint injection in medicine is itself associated with structural harm on repeat dosing - relevant context when a clinic frames a biologic as 'the alternative to steroid shots'.

  7. A four-arm, multicentre, single-blind phase 2/3 randomized trial of 480 knee OA patients (KL II-IV) compared autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction and allogeneic umbilical-cord-tissue mesenchymal stromal cells against a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another, or to the corticosteroid control, and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events occurred.

  8. FORWARD, the longest disease-modifying osteoarthritis drug trial reported to date, gave intra-articular sprifermin (a recombinant FGF-18) or placebo to knee OA patients and followed 378 of them for 5 years. Sprifermin produced a significant, sustained dose-response INCREASE in total femorotibial cartilage thickness versus placebo - and WOMAC pain improved about 50% from baseline in ALL groups, including placebo. It is the cleanest demonstration in the literature that adding measurable cartilage and relieving pain are two different results, and that one does not deliver the other.

FDA states plainly that no stem cell, exosome, stromal vascular fraction, umbilical cord blood, Wharton's jelly or amniotic-fluid product has been approved for the treatment of ANY orthopedic condition - it names osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain and shoulder pain individually. The only FDA-approved stem cell products in the United States are cord-blood-derived blood-forming stem cells for disorders of the hematopoietic system, and there are currently no FDA-approved exosome products.
